ALL FORMS OF PAYMENT ARE ACCEPTED AND ARE DUE AT THE COMPLETION OF SERVICE.

Credit cards will incur an added $3.00 service charge.

There is a $40 fee for returned/non-sufficient checks.

Please contact your insurance provider to learn about your coverage for Acupuncture. I provide receipts to all patients with the appropriate codes to receive reimbursement.  

 

Carefirst, Aetna, United and Cigna recognize the modality and may reimburse up to 80% based on your personal coverage or have a co-pay.  Medicare covers treatments for lower back pain.

Choosing to be unaffiliated with insurance companies, I understand the concern of receiving affordable healthcare.  If you choose to be under my care, we can create a partnership where flexibility is exercised so you receive the treatment you need at a rate that is reasonable for both parties.

Please consider giving 24 hour notice if you need to cancel a session, unless it is an emergency.  There is a late cancellation fee that is accessed based on practitioner and patient agreement.
